Methods and apparatus for performing transluminal and other procedures

The invention is directed to an apparatus for use in a transluminal procedure. The apparatus, comprising, for example, a housing having a guide lumen and a seal proximal to a distal end of the housing that extends across and completely seals the guide lumen; a fixation element in the housing and adapted to secure the distal end of the housing to tissue; and a channel extending through the side wall of the housing having an outlet in communication with the lumen distal of the seal. Methods are also provided. For example, a method includes, performing a transluminal procedure by: securing a datum and position indicator to a wall of a target lumen; forming an opening in the wall; advancing an instrument through the opening; and tracking the advancement of the instrument using the datum and position indicator.

Surgical stapling instruments including a cartridge having multiple staple sizes

A surgical stapling apparatus includes a staple cartridge and an anvil member. The staple cartridge includes a plurality of surgical fasteners disposed in rows of retention slots. The staple cartridge may have an annular or linear configuration of retention slots. The tissue contacting surface of the staple cartridge may be tapered or stepped. The anvil member has a tissue contacting surface that includes a number of pockets arranged for substantially aligning with the retention slots. In addition, the tissue contacting surface of the anvil member may complement the tissue contacting surface of the staple cartridge.

TOOLS AND METHODS FOR EPICARDIAL ACCESS

A retractor tool has a hollow shaft with a proximal end and a distal end. The shaft has an internal bore sized to slidable receive an elongated tool of predetermined dimensions with a distal end of said tool exposed through a distal end of the bore. Such tool may be a visualization tool or other tool used in surgery. A first pivoting jaw and a second jaw are both secured to the distal end of the shaft to pivot about first and second pivot axes perpendicular to an axis of the shaft. The jaws pivot between open and closed positions. In the closed positions, opposing surfaces of the jaws define a distal end of the bore extending completely between the jaws.

Intelligent human-machine interface

Methods and apparatus for an intelligent human-machine interface comprising an interface shell, system agents, function agents, a dynamic documentation system, and a layering architecture. The system agents include dynamic, knowledge-based software object sub-agents that model and track the state of a work area. The function agents model, track, and facilitate work area functions. The interface shell provides a hardware and software interface between the system agents and the function agents. The layering architecture comprises a tracking layer, an equipment and supply management layer, a coordination layer, and a situational awareness layer, and an oversight layer. The oversight layer combines information from the situational awareness layer with process rule sets contained in the function agents to determine if processes are being performed correctly. The interface processes an integrated collection of facts and relationships and recognizes deviation from or compliance with a predetermined process and communicates the same to a user.

Assays and methods using biomarkers

Methods and assays examining expression of one or more biomarkers in a mammalian tissue or cell sample are provided. According to the disclosed methods and assays, detection of the expression of one or more such biomarkers is predictive or indicative that the tissue or cell sample will be sensitive to apoptosis-inducing agents such as Apo2L/TRAIL and anti-DR5 agonist antibodies. Certain biomarkers which may be examined include fucosyltransferases, in particular fucosyltransferase 3 (FUT3) and/or fucosyltransferase 6 (FUT6), as well as sialyl Lewis A and/or X antigens. Kits and articles of manufacture are also provided.

Financial transactions with dynamic card verification values

A payment card comprises an internal dynamic card verification value (CVV) generator and a user display for card-not-present transactions. Card-present transactions with merchant card readers are enabled by a dynamic magnetic array internally associated with the card's magnetic stripe. The user display and a timer are triggered by the user when the user needs to see the card verification value and/or begin a new transaction. A new card verification value is provided for each new transaction according to a cryptographic process, but the timer limits how soon a next new card verification value can be generated.

Methods and apparatus for thermally-induced renal neuromodulation

Methods and apparatus are provided for thermally-induced renal neuromodulation. Thermally-induced renal neuromodulation may be achieved via direct and/or via indirect application of thermal energy to heat or cool neural fibers that contribute to renal function, or of vascular structures that feed or perfuse the neural fibers. In some embodiments, parameters of the neural fibers, of non-target tissue, or of the thermal energy delivery element, may be monitored via one or more sensors for controlling the thermally-induced neuromodulation. In some embodiments, protective elements may be provided to reduce a degree of thermal damage induced in the non-target tissues. In some embodiments, thermally-induced renal neuromodulation is achieved via delivery of a pulsed thermal therapy.
